These forms are a WORD 2003 document. You can complete by using any current version of WORD; or print it on paper and fill in by hand, then mail it in.

To receive a set in the regular mail Registration Packet.

If you are not using Outlook for email then:

• Prepare an email using your system,
• The "to" address is "rollingf@pitel.net",
• Set the Subject to: "MNLA 2011--Request Registration"
• In the Body enter: "Your name, Email address, Mailing address, and (Optional) Phone number"
• Also let us know if you can't open attachments so that the Registration Document can be mailed to you.
• We will email the packet in both "pdf" and "doc" file formats.
• The Player and Character Information forms are also sent in both "pdf" and "doc" file formats. Complete using Word and return as an attachment to an email addressed as above.
• The Player Information form and your check or money order should be mailed to:

• The Character Information form can be mailed to the above address or emailed to rollingf@pitel.net, your choice.
